Median RMD and Total Income

It appears that the RMD is not included in Total Income. Is that correct? If so, what's the definition of Total Income?
That's correct that RMDs are not counted in Total Income.
Total income includes any "Annual Retirement Income" amount from the main input page plus any "income" type cash flows entered in the lower table in the additional inputs window (eg SS, Pension, Annuity, Misc Income).
